Information about the Application Fee:
Applying for an RN/LPN Endorsement and Temporary License can now be done online.  Please note that a $100.00 charge is required if you apply for an Endorsement license, a $40.00 charge is required if you apply for a Temporary license, and a total of $140.00 charge is required if you apply for both Temporary and Endorsement licenses.  All the charges are Not Refundable.

RN/LPN Endorsement and Temporary License Applicants:
You will need a valid e-mail address to be used as your login to the system.  If you do not have an e-mail address, you can apply for an e-mail address from many public WEB sites.  After you create your login, you will register your application information on this WEB site, and we will guide you through every step to complete your RN/LPN Endorsement and Temporary License application.  We also use the e-mail address to send you important information related to your application.
