Applying for a RN/LPN Exam can now be completed online. Please note that a $100.00 fee is required if you apply
for RN/LPN Exam, and is NON-REFUNDABLE.
A valid email address is required. It will be used as your username for this application. Important information
regarding your application will also be sent to this email address. If you do not have an email account, one can
be obtained, free of charge, from many web sites. After creating your login for this RN/LPN application, you will
need to enter additional information about yourself in order to submit this exam request application. The application
will guide you through this process.
IMPORTANT NOTE:
Your license request will not be processed until this on-line application is completed and payment is made.
